SASH Program 

Support and Services at Home (SASH) is a nationally recognized program, shown to improve health, reduce the need for emergency care and hospitalization, and delay or prevent transition to a nursing home or assisted living facility. Participants receive one-on-one support from a SASH coordinator and wellness nurse. Both providers work with a team of community partners to provide comprehensive and coordinated assistance on a regular basis to assist residents in maintaining independence in their homes and enhance their quality of life.

Frequently Asked Questions About The SASH Program

Does it cost money to participate?

  • No. You do not pay anything to participate in SASH, and your insurance is not billed. Ongoing support and care coordination by SASH staff are provided free of charge. Some events offered to the community may involve a fee, but you will know in advance, so you can choose whether you would like to participate. 

    Do I have to sign up for the SASH program?

    • You can enroll, withdraw, and re-enroll at any time. in this voluntary program. You will receive as much or as little support as you want and SASH is always there to support you and your needs. 

    Why should I enroll in SASH?

    • Enrolling will provide assistance that is beneficial to you and your health. A recent federal study showed that SASH participants have much better health and wellbeing. You will have access to your SASH team if health challenges arise.

    Here a few examples of how SASH services can help:

    • We provide a Health & Wellness Assessment, which helps you (and us) know where you may be experiencing challenges and how we can be most helpful to you. We'll then help you put together a Healthy Living Plan, an action plan based on what YOU decide is important, a plan that will help you get to where you want to be.
    • At your request, we'll provide one-on-one support and "check-ins" about the things that matter to you, as well as helpful information and referrals to many health and social services in the community. We can help you connect to specific agencies, classes, health professionals, and others to get you what you need.
    • If you wish, and with your permission, we'll connect everyone on your team, family members, friends, health providers, and others, to make sure they're on the same page. That way, everyone will know how to be most helpful to you, and you won't have to tell your story multiple times. We meet monthly with all of the SASH partners to coordinate any care you may need. 
    • We'll follow you and your health situation if things get tough, if you fall or go to the emergency room, hospital, etc., and make sure your health providers know your needs and concerns so you can get back home as soon as possible. When you return, we'll check in on you to make sure you have what you need.
