Work Order Request

To submit a work order for your BHA-owned or managed apartment, please fill out and submit the following form. Be sure to include a description of the issue, what may have led to the issue, and the nature of the work being requested. BHA property managers or building operations personnel will contact you if additional information is required to address the issue. Not all requests will result in the issuance of a work order and may require further contact before a work order may be issued. This form is only for residents of BHA properties, if you live elsewhere, you will need to contact your landlord directly.

If there is a maintenance emergency, please contact the Building Operations immediately at
802-658-1750 immediately and do not submit an online work order request. Emergency work orders may consist of: smoke alarm not working, no heat, major water leak, toilet overflow,  elevator malfunction, electrical failure, no hot water, broken window or entry door/lock, refrigerator malfunction.  


If you have a life-threatening emergency such as a fire, criminal activity or health matters, call 911 immediately.
